Until recently, individuals with severe and profound disabilities had very
few adult day program options. Traditional program models benefit some
individuals, but not everyone. Pathways is a day program
that enables individuals with severe disabilities to explore, experience and
benefit from social, vocational and educational opportunities. Pathways
offers an alternative to traditional day services, and is also committed to
community integration and exploration in real-life settings.

Every individual is unique, and the
program encourages everyone to express his or her interests, preferences and
needs. Developing and enhancing communication skills, building and
strengthening social and interpersonal skills, interacting with and within
the community, and meeting each individual's goals are all part of the
program.
Ongoing training in a broad range
of areas enhances staff's ability to deliver quality services, and
consultations with various therapists ensure a comprehensive approach to an
individual's program.

Operating with a high
staff ratio, we offer greater opportunity for expressing personal choice.
Staff interactions are based on respect for each individual's opinions,
preferences, cultural identity and right to confidentiality. This
creates an atmosphere conducive to expressing one's individuality and an
understanding for all individuals that their rights are upheld.
Pathways
assists individuals with disabilities to maintain optimal health and achieve the
highest level of independence possible via an interdisciplinary team approach.

Pathways serves adults with developmental
disabilities (including, but not limited to, Mental Retardation, Down
Syndrome, and Autism). Admission is on a case-by-case basis. Entrance
age is established at a minimum of 22 years old. Pathways is open
Monday-Friday, from 9am to 3pm (except on selected holidays, and early
release days). |

Pathways provides both full-time and
part-time day habilitation programming that allows for each member’s
physical impairments, as well as any other limitations they may have.
Impairments and/or limitations our members have include mobility, sight,
hearing, medical and behavioral. Based on each individual’s needs within
the group setting, programming schedules are developed accordingly. |

|
Therapeutic consultative services offered
include: Physical, Occupational and Speech/Communication Therapy.
Services are offered, after initial screening assessments, through
collaboration and consultation with a case manager, developmental
specialist, and registered nurse as deemed appropriate. Pathways also
utilizes a Rehabilitation Aide who acts as a liaison between the
therapists and the staff. |

Personal Care (Activities of Daily Living
or ADL’s), Music, Art, Fitness, and Community involvement are
incorporated, through direct services, into each member’s goals; as well
as included in a member’s regular schedule of activities based upon
their needs and preferences. |
|
|
General medical services, nutrition
issues, and health issues are directly addressed by the program’s R.N.
The R.N. will refer to external services as deemed appropriate. Health
related services include, monitoring of weight and vital signs,
monitoring and observation of health status, and administration and
documentation of medications. |
|
|
Pathways does not discriminate or limit
services based on race, religion, creed, sexual orientation, and looks
to enhance cultural experiences through its diverse programming. |

If psychological boundaries exist, we may
recommend counseling to assist the member with any issues. Additionally,
Pathways staff receive behavioral trainings, have access to an internal
NEV behaviorist, and will work with a behaviorist from the Dept. of
Mental Retardation on a situational basis. |

|
It is the intent and hope of Pathways to
have members excel to the point of transitioning into a more appropriate
setting. However, if that discharge and transition does not occur,
Pathways is prepared to serve all individuals to the best of its
abilities. Any discharge or transition from the program shall be
discussed, at minimum, at a member’s annual team meeting (Refer to
discontinuation of services policy). |

Referrals to Pathways
come from a variety of sources, including the Massachusetts Department
of Developmental Services, the Massachusetts Commission for the Blind,
as well as out of state (non-resident) departments of mental
retardation. Payer sources are Medicaid, the Massachusetts Department
of Developmental Services, the Massachusetts Commission for the Blind,
as well as private payees. Fees are determined by the Massachusetts
Executive Office of Elder Affairs.
